How does botulinum toxin work?
Medicinal product of natural origin used for more than 30 years in medicine, botulinum toxin first treated thousands of patients for medical indications — ophthalmological disorders (strabism), neurological disorders (spasticity, torticolis), dermatological disorders (hypersudation) — before being authorised in France for aesthetics in 2003 under the name of Vistabel. This decrease in safety is unique among injectable care.
The wrinkles of expression result from the hyper-contraction of skinning muscles face. The botulinum toxin temporarily blocks the release of acetylcholine at the neuromuscular junction: the muscle relaxes, the wrinkle resulting from its contraction fades. The expression is preserved — only excessive contraction is neutralized. The doses used in aesthetics are much lower than the usual medical doses.
Dr. Marianne Cayatte dose accurately to never freeze the face. A well-treated patient seems more rested, not frozen. The effect starts at J5–J10, full effect at J15. Retouches are possible at J15 if necessary.

The wrinkles of the upper part of the face do not all form in the same way. Appropriate treatment depends on the cause identified during consultation.
Horizontal ribs of the front
The wrinkles of the forehead result from two distinct mechanisms that must be differentiated before treating:
- Muscle hypercontraction — the frontal muscle contract too often or too loud, creating repeated horizontal folds. Botulinum toxin releases this muscle and erases wrinkles gradually.
- Loss of volume and skin elasticity — the skin has lost its gall; Botox alone will not be enough. One hyaluronic acid painting can be combined to restore hold to the surface of the forehead.
The lens is never a fixed front: the dosage is calibrated to reduce wrinkles while preserving natural expressions. Too much treatment does the opposite effect of what we are looking for.
Wrinkle between the eyebrows: a close look
Wrinkle between eyebrows (or glabella wrinkles) is formed by muscle Corrugator, which pulls the head of the eyebrow inward and downward. Repeated hypercontraction may result in slight sour ptosis and a gradual closing of the eye — giving a severe, tired, sometimes hostile look.
It may appear from 25–30 years in genetically predisposed profiles, or accelerated by tobacco, UV, myopia or high chronic light.
The injection targets the corrugator at about 1 cm from the orbitary edge, with small intramuscular volumes and 2 injection points per muscle. Dr. Cayatte asked the patient to frown during the session to assess the exact tone. The session lasts about ten minutes; some slight redness may appear, without interruption of the usual activities. The area shall not be massaged the following days.

A session in the chair of about 20 minutes, without special preparation, with an immediate return to social life.
Consultation & Analysis

Preparation (optional)
For sensitive patients, an anesthetic cream may be applied to treatment areas 30 to 45 minutes prior to the session. A prescription forArnica Montana is possible upstream to reduce the risk of bruising.
Injection · 10 to 20 minutes
The injections are made in the chair with the thinnest needles on the market (30G), making the act almost painless. The sessions are performed 2 to 3 times a year depending on the metabolism and the areas treated.
Immediate follow-up
Mild redness at the injection site may occur, bruising remains rare. The return to social life is immediate. The first effects are visible at J5–J10, full effect at J15. Accuracy is possible in J15.
Post-injection instructions
These precautions make it possible to optimize the result and avoid any spread of the product:
- 24 hours — no intensive sport
- 48 hours — do not massage the injected areas
- A few hours — avoid putting your head down
- No aspirin or anti-inflammatory drugs in the following week if possible
- No alcohol within 24 hours of the session
The effect improves session after session
After the First injection : average duration of 4 to 8 months. From 2nd injection, the results become more lasting: 6 to 10 months. Regular patients gradually space their sessions.
Dr. Marianne Cayatte recommends not to wait too long before starting — ideally 35 years in prevention. Early injections prevent the furrow from excavating into the skin, even at rest.
What to know before the session
Absolute contraindications: pregnancy, breastfeeding, advanced autoimmune diseases, coagulation disorders, allergy to human albumin.
To be avoided before the sessions: alcohol 24 hours before, aspirin and anti-inflammatory if possible in the previous days (risk of increased bruising).
The botulinum toxin used in aesthetics does not present no serious adverse reactions documented and no reported allergies, due to doses much lower than usual medical doses.
In case of small residual fine lines after the session — linked to an old furrow present at rest — Dr. Marianne Cayatte can treat them by injection of hyaluronic acid or peeling at a follow-up consultation.
